Pathfinders Support Your Path (SYP) Program is an embodiment of our commitment to providing innovative and personalised direct care supports to individuals with a disability. Our primary goal is to help participants embrace life and reach their goals, fostering greater independence while encouraging participation and learning. We are passionate about enabling participants to live their dreams and achieve the very best in their lives. Our dedication extends to community engagement and life skills development, empowering individuals with disabilities and enriching the greater community. 

Pathfinders are providing disability services across the Mid-North Coast and New England North-West.

We offer a range of In-Home and Community Supports designed to empower individuals with disabilities in their homes and communities. Our dedicated support workers provide personalised assistance, focusing on developing essential skills for independence. From daily tasks at home to active engagement in community activities, these supports are tailored to meet individual goals and preferences.

Pathfinders offers a range of home and living support services, including Supported Independent Living (SIL), Short Term Accommodation (STA), Medium Term Accommodation (MTA), and Specialised Disability Accommodation (SDA). Our tailored programs aim to enhance independence, provide temporary respites, support housing transitions, and offer disability-specific living solutions.
