At Pathfinders we offer both Level 2 and Level 3 Support Coordination to help you get the most out of your NDIS plan. Whether your needs are straightforward or more complex, we’re here to guide you with expert, personalised support.

We currently offer Support Coordination in Port Macquarie, Taree, Coffs Harbour, Tamworth, Armidale, Glen Innes, Inverell and across the New England North West. Please reach out to us if you would like to discuss Remote Support Coordination Services.

Level 2 – Support Coordination

This level is ideal for participants who need help understanding and implementing their NDIS plan. Our goal is to support you to build confidence, connect with the right services, and work towards greater independence.

Our Level 2 services include:

  • Helping you understand your NDIS plan and funding
  • Connecting you with providers that suit your needs
  • Coordinating your supports across different services
  • Assisting you to engage with community and mainstream services
  • Supporting you to build skills in managing your own supports
  • Assisting in resolving service issues or challenges that arise
  • Working with you to achieve your goals and outcomes

Level 3 – Specialist Support Coordination

For participants with more complex needs or challenging life situations, Specialist Support Coordination which is funded in your NDIS plan provides a higher level of support. This service is designed to manage barriers, reduce risks, and ensure your services work together effectively.

Our Level 3 services include:

  • Intensive support coordination tailored to complex or high-risk needs
  • Navigating multiple service systems (e.g. housing, justice, health)
  • Crisis planning and resolution
  • Developing service plans that overcome significant barriers
  • Ensuring services are sustainable and meet your long-term needs
  • Assisting you to have greater choice and control.
