Pathfinders Out-of-Home-Care services provide support to young people that require assistance living out-of-home. Depending on their age and needs Pathfinders offers different programs:

Residential Care Program (12 -18 yrs)
Supported Independent Living (18 – 24 yrs)
Interim Care Model (9 – 14 yrs)
Emergency Placement (12 – 18 yrs)

The NPSS program enables children and young people, aged between 0 to 18 years who are living in Out-Of-Home-Care, to interact with their parents and families in a safe and supportive environment.

Supervised visits (also referred to as family time visits) are dedicated times to spend with your child/children under the direct supervision of a trained supervisor.

Pathfinders offers a safe, judgment-free environment for positive family time, while ensuring the safety and comfort of everyone involved.

Sharing knowledge, skills and life experiences, Pathfinders specialised mentors guide young individuals towards achieving their goals. The mentoring program aims to support and encourage vulnerable young people to develop their abilities in their chosen interest area, with the encouragement of a someone with the same passions and experiences.

Therapeutic Care Team is a program to help troubled young people that are dealing with complicated psychological issues resulting in behavioural problems. The aim of therapeutic care is to promote personal change, reflected in changes in thinking, emotional functioning, and behaviour.
