Support Your Path : NDIS

Pathfinders Support Your Path (SYP) is our NDIS-funded disability support program, providing personalised, direct care supports to people with a disability. The program focuses on helping participants build independence, develop life skills and participate meaningfully in their community, in line with their individual goals.

Pathfinders provides Support Your Path services across New South Wales and the Northern Territory, with programs delivered by local teams in each region. Service availability may vary by location. Please contact Pathfinders to confirm supports available in your area.

If you wish to discuss your support needs with a Pathfinders team member, please complete our Expression of Interest (EOI) form and a team member will contact you at your earliest convenience.

Looking for help navigating your NDIS plan? Pathfinders offers Level 2 and Level 3 Support Coordination to help you access the services you need and build the skills to manage your supports with confidence. Now available across Port Macquarie, Taree, Coffs Harbour, Tamworth, Armidale, Glen Innes, Inverell, and the New England North West.

We offer a range of In-Home and Community Supports designed to empower individuals with disabilities in their homes and communities. Our dedicated support workers provide personalised assistance, focusing on developing essential skills for independence. From daily tasks at home to active engagement in community activities, these supports are tailored to meet individual goals and preferences.

Pathfinders offers a range of home and living support services, including Supported Independent Living (SIL), Short Term Accommodation (STA), Medium Term Accommodation (MTA), and Specialised Disability Accommodation (SDA). Our tailored programs aim to enhance independence, provide temporary respites, support housing transitions, and offer disability-specific living solutions.
