Support Your Path (SYP) reflects Pathfinders’ commitment to innovative, person-centred disability support. We support people with disabilities to build independence, participate in their communities and work towards their personal goals and dreams.

About the Role

We are seeking passionate Support Workers to join our Support Your Path team on a casual basis. You will work one-on-one or in group settings to deliver individualised supports aligned with participants’ NDIS plans.

Key Responsibilities:

  • Providing safe, nurturing and person-centred supports
  • Assisting with daily living tasks, personal care and domestic assistance
  • Supporting participants with health, wellbeing, community access and recreational activities
  • Maintaining accurate case notes, timesheets and documentation
  • Working across a rotating roster including afternoons, nights, weekends, sleepovers and public holidays
  • Supporting participants with complex needs, including epilepsy and challenging behaviours

About You

You are caring by nature, flexible, and passionate about empowering people with disabilities to live meaningful lives. You value professional boundaries, ethical practice and teamwork.

Essential Criteria:

  • Relevant tertiary qualification in Human Services or willingness to obtain, and/or demonstrated experience
  • NDIS Worker Screening Check (or willingness to obtain)
  • Ability to provide personal care and domestic assistance
  • Strong commitment to human rights and person-centred practice
  • Availability to work a range of rostered shifts, including short-notice shifts if required
  • Excellent written and verbal communication skills
  • Ability to work independently and as part of a team
  • First Aid and CPR (or willingness to obtain)
